CONSENT FORM AND RELEASE FOR WATER METER REPLACEMENT

PROOF OF CORRECT FORM FOR SERVICE LINE REPAIRS

As a property owner it is important to keep your property side plumbing in good working order. Why?

  1. The Water Department needs to be able to conduct necessary water meter repair or replacement
  2. You don't want to spring a leak that requires costly emergency repair

Property owners must maintain everything after the water meter (S.C.M.C. 16.04.070)

If your plumbing looks anything like the examples below, or you know you have a leak on your service line, consider calling a plumber.

If you have already received a letter from Santa Cruz Municipal Utilities regarding the condition of your service line, you have the option to complete an online or paper Consent Form and Release for Water Meter Replacement.

If you have opted to repair or replace your water service line, please complete a Proof of Correction.

Completed paper forms can be mailed or brought to:

Where to Start

  1. Visually assess the condition of your service line by opening your water meter box. Typically your box will be located in front of your house  in your front yard or the sidewalk. If you can't find your box, please give us a call and we can help you locate it.
  2. If you see severely rusted, corroded, or cracked plumbing on your side of the water meter, consider calling a plumber. You may need to repair or replace your service line.
